Hair transplantation involves removing small pieces of hair-bearing scalp grafts from a donor site and relocating them to a bald or thinning area. There are different methods of Surgical Hair Transplantation, such as scalp reduction, tissue expansion, strip grafts, scalp flaps or clusters of punch grafts.

Hair Transplant By Strip Method

The Strip Harvesting technique is a hair transplantation method that involves removing the follicular units from the donor area as one group. This hair transplant technique allows the follicular units to be extracted intact in their genetic form. Each follicular unit contains one to four hair, the average being 2.2 hair per follicular unit. An integral part of the process is to receive follicular unit grafts that have not been damaged. The harvested follicular units are then dissected with great care under fibre optic equipped microscopes to avoid the application of heat that may harm the hair follicles. Once separated, these follicular units are preserved in a special saline solution at the correct temperature waiting to be placed in the recipient area, which is the bald or thinning area on the scalp. Continuing the  process, the surgeon first prepares recipient sites on the patient. The final step of the  surgery is the follicular unit placement.

Follicular Unit Extraction

Hair Transplantation by the Follicular Unit Extraction method (FUE) was considered to be a breakthrough in the hair transplant industry when it was commercially introduced in the western world in 2002. In FUE, the extraction of intact follicular unit is dependent on the principle that the area of attachment of arrector muscle to the follicular unit is the tightest zone. In this transplant procedure the donor area is required to be fully exposed by shaving it down to zero.

If the patient however needs up to 1500 grafts, then the donor area does not need to be fully shaved and sufficient harvesting area can be exposed by a marine “crew” cut. The surgical procedure starts with administration of the local anesthesia to the donor area and the extraction of the follicular units. A simple way of presenting how FUE follicular units are extracted is shown in the figure below. FUE has one main advantage- the absence of any linear donor site scar, thus permitting the patient to shave his head if desired. Moreover there is almost limitless donor, especially if body hair can be used. The extracted FUE follicular units are passed to the technicians of the medical team to check that the quality is of the highest standards under the microscope and immediately after group them into single, double, triple and quadruple follicular units. The final stages of the FUE  procedure are the creation of the recipient sites by the doctor and the placing of the follicular units by the rest of the medical team.

Which method is best for you?
Both the FUE and the Strip Excision procedures have advantages and disadvantages. The FUE procedure, given its minimally invasive nature, does not produce one long linear scar but rather hundreds of tiny scars in the donor area that are often hard to detect. This has fueled interest in the FUE procedure with patients who are highly concerned about potential visible scarring in the donor area.
